Hello, my name is Annie Oakley. My animal id is #12744. I am a male gray tabby kitten at the Manhattan Animal Care Center. The shelter thinks I am about 4 months old. – P
I came into the shelter as a stray on 15-Nov-2017.
Annie Oakley is at risk due to having an Upper Reapiratory Infection as well as being dehydrated and emaciated. She will likely require in home care with close monitoring of her condition. There are no known behavior concerns for her at this time, average determination.

[LVT Intake Exam] Microchip Scan: negative, placed 981020021756061 Evidence of Cruelty: no Observed Behavior: allowed all handling, friendly but disoriented due to health Sex: neutered male Estimated Age: appx 4-5m Subjective: young cat appears to have a URI, skin on planum is sloughing off and area on lip just below nose is torn as if from rubbing, live fleas seen Eyes: heavy ocular d/c that refills easily when cleaned and when bridge of nose is touched (sinus infection?) Ears: clean Oral Exam: pale mm, young, permanents in Heart: WNL Lungs: WNL, sneeze when touched Abdomen: WNL Musculoskeletal: underweight BCS 3/9 Mentation: QAR, dehydrated Preliminary Assessment: possible URI, flea infestation Plan: 25ml LRS SQF, washed face after using warm water compress, DVM intake
Placed 22G IVC RFL. IVF initiated at 15mls/hr. Administered Erythromycin 0U, Doxy suspension 0.3mls PO, Simbadol 0.2mls SQ.

Scanned neg – MC placed History : Police intake – stray cat with URI Subjective: Dull, lying down until stimulated Observed Behavior – Allows all handling Objective Quiet, estimated 8-10% dehydrated, BCS 2-3/9, MMs pink EENT: OU enophthalmia, moderate mucoid ocular discharge. Moderate mucoid nasal discharge. Ears clean. Oral Exam: Teeth clean but gingival inflammation and possible ulceration at the front of the mouth H/L: NSR, NMA Lungs clear, eupnic ABD: Soft, non painful, no masses palpated U/G: Male intact kitten, two testicles palpated MSI: Ambulatory x 4, no masses noted, healthy hair coat CNS: Slightly depressed but appears mentally appropriate Rectal: Normal externally Assessment: 1. Emaciation, dehydration 2. URI, oral ulceration Prognosis: Good Plan: 1. Blood drawn for CBC/chem – running now 2. Place IVC and start IV LRS at 15 ml/hr for 12 hours, reassess rate overnight 3. Doxycycline 0.3 ml PO SID x10 days 4. Erythromycin OU BID x10 days 5. Nebulize BID x5 days 6. Simbadol 0.2 ml SQ SID x4 days SURGERY: Temporary waiver due to URI and emaciation 1088
CBC results: Normal Hct (42%) Normal WBC (13.8), normal diff Low Plt (R/O machine error) Chemistry results: Mild hyperglycemia (159) Azotemia (55) T. protein WNL (8.2), alb WNL (2.8), globs elevated (5.4) GGT elevated (4), t. bili elevated (1.2) Normal ALT, alk phos Na and Cl elevated (Na 168, Cl 131) Hyperglycemia – R/O stress, infection Azotemia – R/O renal vs. pre-renal Elevated GGT and t. bili – R/O GI upset, mild hepatocholangitis Continue treatments as scheduled, rec repeat chem in 1-3 days. 1088
Hx: URI, emaciation. On IV LRS at 15 ml/hr. Pt developed fat paw – tape for IVC was cut and bridged. So sweet! Pt making muffins and purring while lying down, curled up. Estimated 5-8% dehydrated based on decreased skin turgor. Mild serous ocular and nasal discharge. Soft abd, amb x4, no skin lesions noted. Continue treatments as scheduled, re-evaluate hydration tomorrow – consider removing pt’s IVC then. 1088
Recheck in med-iso for URI, poor BCS, dehydrated: S/O: QAR, still mildly dehydrated – perks up when played with, friendly, good appetite for wet food – IV cath in place/patent but paw remains mildly swollen – decr fluid rate to 10 ml/hr – severe oral ulcerations, mild sneeze, serous nasal discharge A: condition stable/improving P: cont. tx – recheck cath replace/remove tomorrow
Oral ulceration, dehydration on intake; catheter not patent this morning and was removed S/O -BAR, very affectionate and interactive; docile, allows all handling -mm pk, moist; small area of ulceration along L dorsal aspect of tongue and along R caudal aspect of tongue -mild serous nasal discharge -OU: open and clear -eupnic, low purr on auscultation -soft abdomen -small wound on R distal antebrachium with mild discharge dried in fur, no discomfort or lameness A Oral ulceration-stable/improving Small wound on RF P extended simbadol recheck in 3 days

Behavior Condition: 1. Green
Annie allowed to be picked up and held. Her body was relaxed and collected. Her eyes were shut due to discharge. She stood in one place and was not interested in wet food. The admissions counselor was able to collar her and take her in and out of the carrier.
Basic Information:: Annie Oakley arrived to MACC with the NYPD. She was brought because she appeared sick. She is approx. a female gray tabby and approx. 7 months old.
KNOWN HISTORY:: Annie was brought in as a stray, so we don’t have any behavioral history or tendencies in a home environment.
MEDICAL BEHAVIOR:: Perks up when played with, friendly
ENRICHMENT NOTES:: Resting at the front, soft eyes and body. Seems really rundown, lethargic, eyes crusty. Lay in place when door opened, started to lean and arch into pets. Seemed enthusiastic, despite low energy. Rubbed against bars after door closed.
Cage Condition:: No change
Reaction to assessor:: Annie remains neutral, crouched position on her cage bedding during the approach.
Reaction when softly spoken to:: Annie responds back with a soft meow.
Reaction to cage door opening:: Annie remains relaxed, joyfully rolling over, alert with normal shape eyes.
Reaction to touch:: Annie leans in to the assessor’s hand and purrs while appreciating the petting.
Reaction to being picked up:: Allows the pickup and remains calm.
ACTIVITY LEVEL:: Mellow
VOCAL:: Somewhat chatty
CHARACTER TYPE: : Calm,Sweet
