Medical Summary DVM Intake Exam Estimated age: 1-2yrs Microchip noted on Intake? no History : O/S due to too many cats. Subjective: Cat is BARH. Observed Behavior – Shy/timid at first. Keeps to back of cage, but peeks through opening to eat food on other side. Pushes into pets. Allowed all body parts to be touched without signs of discomfort. Purring. Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = wnl R = wnl BCS 4-5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Grade 0-1/4 dental dz PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, abdomen noticeably distended, palpable fetuses. U/G: wnl MSI: Ambulatory x 4; cat has wide based stance in the hind and seems to have an abnormal gait, but hard to assess in cage, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Pregnant, Possible gait abnormality Plan: Placed through NH and scheduled for spay at Glendale tomorrow. Acceptable candidate for elective surgery. Prognosis: Good SURGERY: Okay for surgery
