Medical Summary 12/11/16 13:14 BAR. Friendly but nervous, allowed all handling ~6% dehydrated BCS 4/9 Ate baby cat EENT: Excess yellow purulent bilateral nasal discharge. Nasal congestion audible. Severe yellow mucoid ocular discharge OU. Eyes hyperemic with keratitis affecting full cornea OU + neovascularization; elevated nictitans OU. Suspect limited vision – possibly sees shadows. Moderate debris AU Adult canines/incisors; clean. Oral WNL. MM pk/<2 Patch of crusted alopecia at R temporal region (1.5 cm diam) – fluoresces under Woods Lamp Flea dirt throughout coat – no live fleas observed Grade III/VI left parasternal HM; NSR. Lungs clear – referred upper airway sounds; normal BVS Abd snp/nmp Amb x4; no injuries evident A: ~6 mo FI DSH 1. Severe URI 2. Suspect herpetic keratitis OU causing visual impairment with secondary bacterial infection 3. Heart murmur 4. Suspect ring worm 5. Flea dirt 6. Mild dehydration P: Move to isolation (URI + RW suspect) Gave 75 ml SQ fluids Bayril 5 mg/kg – first dose given SQ. Tx x 10 days DTM + weekly lyme treatment Nebulize BID x 3 days Terramycin BID x 5 days Recheck 2 days Needs picture Deworm + Flea treat Prognosis good overall with treatment; Px poor for visual gains
