Medical Summary DVM Intake Exam Estimated age: 5 years Microchip noted on Intake?neg History : owner died; brought in with another persian (Cappucino A1110057) Subjective: QAR Observed Behavior – nervous but very sweet; allows handling, examination and nail trim Evidence of Cruelty seen – Evidence of Trauma seen – Objective T =not taken P =WNL R = WNL BCS 4/9 EENT: mucoid ocular discharge, mild chemosis OU; mild corneal opacity in mid field OS-suspected scar serous nasal discharge, sneezing during exam AU-mod waxy debris, especially on ear pinna Oral Exam: mm pk, moist; CRT <2 sec; mod calculi build up and gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted nails on digit III on RF and digits II-IV on LF grown into pads, clipped nails but has bleeding and irritation of digital pads CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment 1. URI with conjunctivitis 2. moderate dental disease 3. nails grown into pads with irritation 4. waxy aural discharge Plan Clipped nails, soaked paws in nolvasan move to iso doxycycline x 10 days erythromycin x 7 days flush ears rec’d dental prophylaxis Prognosis: good to excellent SURGERY: neutered
