11/23/16 11:25 Ali is an Orange tabby that is a 14 years old. The owner received Ali 2 weeks ago from a neighbor. The owner suffers from allergies and can’t care for Ali. Ali has no known illness. Ali has been introduced to small children ages 4. He keeps to them self and does not solicit attention. He stares and sits in high top area. Ali displays the same behavior around stranger. But he slowly allowed warms up and solicits attention. Ali has lived with a small dog and they were respectful of each other’s space. Ali has lived with another cat in his previous home. The cats engaged in play and lounged around the house together. Ali allows being petting and holding for a few. He has never received a bath, has his nails trimmed or has his coat brushed. Ali has never had a bite history or scratch history. He walks into his carrier but meows and cries in the carrier. He always uses his hooded litter box that was located in a spare room of the house. Ali is described as mellow, friendly and independent. His favorite activity involves sleeping and eating. He also enjoys looking out the window. He enjoys a diet of Wellness dry food. Ali sleeps in the owner’s bed and allowed to be petted on his back. Ali during intake was meowing and crying. But he walked out the carrier nicely, allowed to be petted. He allowed to be collard, scanned and photographed.

As per previous owner, Ali has been introduced to small children ages 4 years old and would keep to himself and wouldn’t solicit attention. He displays the same behavior around strangers but takes time to warm up to them. Ali has lived with a small dog and they were respectful of each other’s space. He lived with another cat in his previous home and would engage in play and lounged around the house together. His owner describes him as mellow, friendly and independent. Reaction to assessor: Ali gets up out of his bed, meows loudly then comes over to greet the assessor. Reaction when softly spoken to: Ali rubs against the kennel door , reaches out with paws and continues to vocalize. Reaction to cage door opening: Ali is calm and relaxed. Reaction to touch: Ali meows loudly until you start interacting with him then he gently leans in for rubs and appreciates attention. His body relaxes, he brushes up against the assessor, allows petting all over and is very affectionate. Reaction to being picked up: Ali was a bit tense when held, lip licks then looks around scanning his environment. Behavior Determination: Average Ali interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.

Scanned negative. BARH – Very vocal. Purrs, allows lifting, handling, combing. Resisted nail trim. “Cranky senior” N~14 years Ears – dried, yellow-beige crust removed. Very itchy. No further seepage. No parasites under microscope. Possible otitis? VC requested. Eyes cloudy Nose, throat clear Teeth deeply stained, grooved, worn but in good condition for age. Coat clean but thick. Not matted but needs grooming along lower back and base of tail. Began combing out excess fur – cat enjoyed the attention. No flea dirt or parasites noted Ambx4 Nails long – resisted trim, tried to flee. Not completed. NOSF
12/07/2016 RE-EXAM (LAST MAJOR EXAM)
Medical rating 3 C – MAJOR CONDITIONS , behavior rating AVERAGE
12/07/16 URI noted on rounds S/O: Q/BAR. Attention seeking. Eating well. No vomit EENT: Mild mucoid nasal discharge H/L: Sneeze marks in cage, slight congestion A: URI, geriatric P: Rec doxycycline 1.1ml po q24 x 10 days. Good prognosis 12/03/16 Vomiting reported on symptoms log yesterday, monitor S/O: BARH. Attention seeking. Appetite ok. Clean litter. No vomit seen or reported today. EENT: Nuclear sclerosis, no ocular or nasal discharge A: Geriatric, 1-2 episodes of vomiting yesterday P: Continue to monitor while at BACC. Good prognosis
