Progress exam Subjective: QAR. Very nervous in the kennel. No c/s/v/d but has serous ocular d/c and crusts around eyes. No nasal d/c. Had swelling above left eye on intake. Good appetite. Normal bm and u in litter box. Objective EENT: Serous ocular d/c with mucoid dried crusts OU with OS>OD, no nasal discharge noted H/L: eupneic, normal RR/RE, no c/s MSI: Ambulatory x 4, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Conjunctivitis OU Plan: Continue to monitor while at BACC Start erythromycin OU BID x5d until 1/7 Recheck 1/7 Prognosis: Excellent

KNOWN HISTORY:: Alexis was brought in as a stray, so we cannot speak to his behavior in his previous home.
MEDICAL BEHAVIOR:: Stiff, fearful, non-aggressive, will flee
ENRICHMENT NOTES:: 12/28/17 Crouched in back of kennel, body and face tense. Tolerates petting along head and body with a slow approach while leaning towards portal, tail low and wrapped around body and eyes squinted shut. Nervous, needs more time to adjust. 01/02/18 Hiding beneath kuranda bed, pupils wide and tail wrapped around body. He steps forward to sniff my hand, then tolerates petting along his head and body while retreating slowly to the other side of the kennel with his tail down. Not interested in treats at the moment. Needs more time to adjust. 01/03/18 Lying on top of kuranda bed, body and face tense. Makes eye contact and hisses when spoken to, ears flattening. Hisses when I place treats next to him. He leans away from my hand when touched and slowly turns his head to face the wall. Nervous and uncomfortable, needs more time to adjust
Cage Condition:: Cage is neat
Reaction to assessor:: Alexis was tense and sitting up underneath his kuranda bed with his tail wrapped around his feet.
Reaction when softly spoken to:: Alexis lip licks, then slowly turns his head away and faces the wall.
Reaction to cage door opening:: Alexis retreats to the other side of the double kennel.
Reaction to touch:: Alexis shifts away and tries to dodge the assessor’s hand when attempting to make contact. He flinches slightly when touched, closes his eyes and pauses as the assessor pets him along his body. His ears bend flat and he tolerates brief contact before shifting away.
ACTIVITY LEVEL:: Laid back
VOCAL:: Quiet

POTENTIAL CHALLENGES:: Fearful
Potential challenges comments:: Alexis has displayed fearful behavior during their stay in the care center and may dislike certain types of handling. A fearful cat will feel more relaxed when given options, so provide him with the chance to move closer, investigate, or interact with you. Be sure to offer incentive such as treats or play time whenever the cat makes a small positive step. Please speak to an adoption counselor for additional information on methods to desensitize your cat to their fear stimulus.

BEHAVIOR SUMMARY:: Alexis tolerates attention and petting but may be fearful or stressed in the shelter, and may be intimidated by small children. He may be a little more independent, and may need time to warm up to his new home. Due to the behaviors seen in the care center, we feel that this cat will do best in an experienced, adult only home.
