I came in the shelter as a OWNER SUR on 01/27/2017 from NY 11421, owner surrender reason stated was OWNER HOSP. I came in with Group/Litter #K16-084851.

01/31/17 Nasal discharge noted on rounds. O: QAR. Small amount of canned food consumed. Resting on bedding, but flees to back of cage when door opened. Full exam not performed. EENT: Mild mucoid nasal discharge, small amount of sneeze marks in cage. No congestion, no ocular discharge. A: URI. P: Move to iso. Discontinue Clindamycin for now. Rx. Doxycycline 50 mg/ml. 1 ml Po q 24 hours x 10 days. Good prognosis.

Unknown Previously lived with: __ adults and __ children ages ___ Unknown Behavior toward children: Unknown Behavior toward strangers: Unknown Behavior toward cats: Unknown Behavior toward dogs: Unknown Bite history: Unknown Scratch history: Unknown Litter box training: Unknown Energy level/descriptors: Unknown Other notes: Was removed from a home after owner was hospitalized; had, apparently, been living with two other adult cats. EVALUATION: Cage Condition: No change Reaction to assessor: Remains lying low in rear corner of cage, tense body posture Reaction when softly spoken to: Doesn’t move, makes some eye contact Reaction to cage door opening: Doesn’t move, but body posture tenses further, makes some eye contact Reaction to touch: Able to touch head, but pulls away from my hand and hisses, doesn’t allow further touching ENRICHMENT NOTES: 01/29/17 Arrived from same home as Ashton, 1102351, and Jewel, 1102349, each in his/her own cage. Tense and wary, pressed into a rear corner of his cage, trying to be invisible. Not receptive to touching or handling, backs away from the attention. Needs time to adjust. Sprayed a little Feliway, left some treats and a toy. Adding to socialization list. 01/30/17 Body and head pressed low, against floor of kennel, in a rear corner. Fearful, not adjusting well so far. Not certain how much being separated from his former housemates is affecting him. Resists touching, pulls back and may hiss but does not bite or swat. Showing interest in chicken when offered, will sniff, but didn’t eat with me there. Sprayed some Feliway. Needs time to adjust. Scan : Negative (Housed with A1102349, A1102351) Sex: Male neutered. Approx 3 yo – based on size/ appearance. S) Tolerates handling somewhat. Will flee. But no growling or aggression. Blood noted on chin. O) QAR. MM pk. Hydrated. EENT: Eyes – OD moderate MPOD medial canthus, marked scleral hyperemia, mild chemosis, mod conj hypermia; OS – mod scleral hypermia but no d/c. Ears clean. NND. Oral: Dried blood on chin, rostral mandibular and maxillary gingiva mildly swollen. Ulcer on tip of tongue. Minimal tartar but marked gingivitis with purulent generalized d/c in mouth, marked halitosis. MSI: BCS = 5/9. Ambx 4. Good haircoat. No fleas/ wounds seen. Some dried blood on paws but no wounds – likely rubbing mouth CV: HR= 200, NSR, NM. L: clr bilat and eup. Abd: SNP, NMP. UG: neutered male A) 3 yo MC DSH Dental dz – r/o suspect calici (housed in multi-cat household- A1102349 (also FIV+) and A1102351 have ddz also) vs nutritional/ other. Ocular d/c and hypermia likely due to underlying ddz vs other infxn. P) Pen G 0.5 cc – for oral dz. Clindamycin 25 mg/ ml 2 cc po sid x 10 days – start tmrw. Cleaned eyes and placed terramycin OU once but will monitor how eyes respond to oral abs. Monitor app. Re-check in 2 days. PROGNOSIS: GOOD.
