History : Patient is stable.
Subjective: Underweight intact female with alopecia.
Observed Behavior – Friendly before venipunture. After venipucture the patyient started scratching.
Evidence of Cruelty seen – NAF
Evidence of Trauma seen – NAF
Objective
P160 bpm BCS 4/9
EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: NAF
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Intact female
MSI: Ambulatory x 4, skin free of parasites, no masses noted, alopecia under the jaw.
CNS: mentation appropriate – no signs of neurologic abnormalities
