Medical Summary DVM Intake Exam Estimated age: 3 YO MC DSH Microchip noted on Intake? neg on intake History : owner surrender Subjective: BARH, friendly, allows all handling (stays completely still for exam/procedures) Objective BCS: 5/9 EENT: Eyes clear, ears flaky brown/black debris with licehnified skin, really heavy chunky light brown colored debris deep in ear canal, no nasal discharge noted – skin of nose hyperemic/flush (likely normal for this patient) Oral Exam: nice teeth, no oral masses PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MC MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy long hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: grossly normal Assessment: adult MC DSH 1. otitis externa (presumptive) Plan 1. cleaned ears on dvm intake 2. RX: tresaderm OU BID x 10 days Prognosis: excellent SURGERY: already done
