Hello, my name is Adley. My animal id is #10302. I am a desexed female tortoiseshell cat at the Brooklyn Animal Care Center. The shelter thinks I am about 14 years 1 weeks old. – P
I came into the shelter as a stray on 20-Oct-2017.
Adley is at risk due to being diagnosed with an Upper Respiratory Infection and will likely require home rest and a series of antibiotics for up to 14 days. This is a contagious illness to other cats.

DVM Intake Exam Estimated age: 12-14 years Microchip noted on Intake? negative History : stray Subjective: BARH Observed Behavior – hisses and swatting, feet seem painful Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = eupneic BCS 6/9 EENT: Eyes clear with nuclear sclerosis ou, ears clean, no nasal discharge noted Oral Exam: adult dentition, Small soft tissue mass on lower left gum ~0.25cm PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: FS, no MGTs, no vulvar d/c MSI: Ambulatory x 4, skin free of parasites, matted and unkempt haircoat, nails severely overgrown with some embedded into pad (RF 2&5, LF 3) CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Geriatric Nuclear sclerosis ou Severely overgrown nails Embedded nails-right front 2&5, Left front 3 Matts Small soft tissue mass on lower left gum ~0.25cm r/o neoplasia Plan: Continue to monitor while at BACC Trimmed nails Telazol 0.1ml IM for PE Shaved matts Rec geriatric workup with placement Rec histopath of mass on mouth Soak feet in dilute chlorhex One dose simbadol 0.24mg/kg SQ Recheck nails in 3 days +/- add abx (no convenia available) Prognosis: Good SURGERY: spayed
Recheck embedded nail site History : stray intake 10/20-had embedded nails (clipped and scrubbed), matts (shaved), and a mass on her mouth Subjective: BARH. No c/s/v/d. Hissing with PE. Objective BCS 6/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: adult dentition, Small soft tissue mass on lower left gum ~0.25cm H/L: eupneic ABD: Non painful, no masses palpated, distended U/G: FS, no MGTs, no vulvar d/c MSI: Ambulatory x 4, skin free of parasites, shaved haircoat in some patches on dorsum, nails trimmed, previously embedded nail wounds are healing well with small scabs and no d/c CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Geriatric Nuclear sclerosis ou Severely overgrown nails-trimmed on intake Embedded nails-right front 2&5, Left front 3-wounds healing well Matts-shaved on intake Small soft tissue mass on lower left gum ~0.25cm r/o neoplasia Plan: Continue to monitor while at BACC Rec geriatric workup with placement Rec histopath of mass on mouth Prognosis: Good
Monitor for URI S/O: QAR. Food available, appears untouched EENT: Nuclear sclerosis, epiphora OU, serous nasal discharge HL: Normal RR/RE, sneeze marks in cage A: Geriatric, URI, embedded nails and oral mass noted on intake P: Rec doxycycline 1.3ml PO q24 x 10 days, scheduled blood work for tomorrow. Poor long term prognosis
CBC: leukocytosis 22.22 (2.87-17.02) with neutrophilia 18.99 (1.48-10.29) with suspect bands, thrombocytopenia likely due to clumping Chem: hyperglobulinemia 5.8 (2.8-5.1) and mild hyperglycemia 184 (71-159) T4: wnl
CBC/Chem, Lites, T4 run. Unable to palpate bladder; animal is extremely overweight. Vet 1416 notified

KNOWN HISTORY:: Adley was brought in as a stray, so there is no information on their behavior history or tendencies in a home environment. She allowed her finder to handle her without issues. During intake, Adley came out of her carrier and allowed the counselor to pet her and handle her.

ENRICHMENT NOTES:: 10/22/17 Lying in back of kennel, body soft and ears forward. Makes eye contact. She allows petting along her head and body while leaning in and stretching. Kneads kennel floor with one paw. Sweet senior lady – did well today!

Reaction when softly spoken to:: Adley does not respond and is fast asleep.
Reaction to cage door opening:: Adley remains motionless and continues to sleep.
