Medical Summary DVM Intake Exam Estimated age: 4-8 years based on dentition and overall appearance Microchip noted on Intake? Scanned negative History : Finder has been feeding this cat since about February 2017. For the last 3-4 months, pt has been losing weight and looking ill. Subjective: Alert, looks around, tries to run away but when restrained with hands – pt allows handling. Objective BAR, estimated 8-10% dehydration based on severely decreased skin turgor and enophthalmia, BCS 3-4/9, MMs pale EENT: Enophthalmia OU. Mild mucoid discharge OU. Clean ears. Moderate seromucoid nasal discharge and a nasal planum ulceration. Oral Exam: Moderate tartar and gingivitis PLN: No significant enlargements noted H/L: NSR, NMA, Regular sneezing. Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female, presumed intact MSI: Fur coat needs grooming – some small chunks falling out. Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. Declawed x4 feet. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment 1. Dehydration 2. URI and conjunctivitis 3. Weight loss – R/O cachexia due to underlying illness vs. difficulty finding food due to being declawed Plan: 1. CBC/chemistry/combo test 2. LRS 250 ml SQ during exam 3. Move to med iso – consider placing IVC 4. Doxycycline 0.8 ml PO SID x10 days 5. Erythromycin OU BID x10 days Prognosis: Fair SURGERY: Temporary waiver due to URI 1088

8/3/17- Resting towards back, soft eyes and body. Lethargic, clearly not feeling well. Lay in place when door opened, leaned slightly into pets, very sleepy.
Cage Condition: No change
Reaction to assessor: Addison remains neutral, lying down on her cage bedding during the approach.
Reaction to door opening: Addison remains in place, relaxed body posture, alert with normal shape eyes.
Reaction to touch: Addison head bunts the assessor’s hand and appreciates petting on the head and body.
Reaction to Being Picked up: Allows the pickup and remains calm.
BEHAVIOR SUMMARY: Beginner
Addison interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting.
This cat can go to a beginner home.
