I came in the shelter as a OWNER SUR on 04/02/2017 from NY 10458, owner surrender reason stated was HOARDING. I came in with Group/Litter #K17-092789.

04/06/2017 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 8.1 LBS.
4/6/17 1:50 pm S: QAR, nervous but sweet O: no further seizure activity EENT: No discharge OU, AU, nose; mm pk/moist PLNs: Not enlarged. H/L: NSR, NMA, SSP M/S/I: Amb x4. No skin lesions noted. UG: Male neutered, scrotum small and soft, no discharge Neuro: alert; pupils dilated but responsive to light; pos PLR/meance OU; intermittently dec CP on FL but suspect behavioral; normal withdrawal x 4 Rectal: Not performed. A: Seizure-r/o epilepsy vs other P: start keppra 20 mg/kg TID Hx: Was being transported up to adoptions in a carrier about 20 minutes ago, when pt had a seizure. Per LVTs and assistants who witnessed the seizure, pt had generalized tonic clonic movements, urinated and defecated, and the seizure lasted for MORE than 60 seconds. S: Sitting, tachypneic, did not move around but very alert and followed my movements with his eyes. Allowed all handling. O: BAR-H, BCS 5/9, MMs pink and moist. Temp 101.6. EENT: No discharge OU, AU, nose. Clean teeth. PLNs: Not enlarged. H/L: Tachycardic – HR 220 bpm. NSR, NMA. Tacnypneic – RR 48 breaths/min. Quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. No skin lesions noted. UG: Male neutered, scrotum small and soft, no discharge Neuro: Alert, follows all movements in front of him with his eyes. Very passive. Rectal: Not performed. A: Recent very long seizure – R/O true seizure vs. cardiac event vs. respiratory distress vs. other event Short-term prognosis: Good-fair P: Run CBC/chemistry. Keep in Medical. If future seizures, okay to give diazepam 0.35 ml IV, or 1 ml per rectum. 1088

KNOWN HISTORY: Lived Indoors ENRICHMENT NOTES: 4/3/17 – Hunched in litter box, tense, alert. Eyes became dilated when door opened. Tolerated pets, but leaned away and started retreating to the other kennel. EVALUATION: Cage Condition: Cage is slightly re-arranged Reaction to assessor: Adam looks tense, crouched position when approached by the assessor. Reaction to cage door opening: Adam cowards away with tail curled close to the body. Reaction to touch: Adam sits up in place then leans away from the assessor’s hand, but once he accepts the stroke he stiffens up with no interest. ACTIVITY LEVEL: Motionless VOCAL: Quiet CHARACTER TYPE: Shy and Timid RECOMMENDATIONS: – Experienced cat parent Adam tolerates attention and petting but may be fearful or stressed in the shelter. We recommend that this cat go to a home with experienced cat parents.
