Medical Summary DVM Intake Exam Estimated age: Reported to be 2 years old, exam is consistent with this Microchip noted on Intake? Scanned negative – placed MC during exam History : Surrender. Small wound on scrotum. No other medical issues. Subjective: Friendly – approaches for petting, explores the table, allows all handling Objective BCS 5/9, MMs pink and moist EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Mild tartar and gingivitis PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male intact, testicles soft and symmetrical. MSI: 1×2 cm thickened erythematous region on the right side of the scrotum, half on the scrotum and half off. Mild serosanguinous discharge. Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: 1. Wound on right side of scrotum – R/O trauma as original cause but pt may now be licking it 2. Mild tartar Plan 1. Temporary surgical waiver – start amoxiclav 45.7 mg/ml 1.2 ml PO BID x 10 days. If not resolved in that time, consider biopsy of region. 2. Recommend regular dental care including daily tooth brushing and routine cleanings Prognosis: Good SURGERY: Temporary waiver due to WOUND ON SCROTUM 1088
