Oral Exam: kitten dentition, no oral lesions noted
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic
ABD: Non painful, no masses palpated
U/G: FI, no MGTs, no vulvar d/c
MSI: Ambulatory x 4, skin free of parasites, no masses noted, patchy alopecia and dirty haircoat
CNS: mentation appropriate – no signs of neurologic abnormalities

Plan: Continue to monitor while at BACC
Wood’s Lamp positive
Start gentamicin OU BID x7d until 5/13 (first dose given)
Start clavamox 13.75mg/kg PO BID x7d until 5/13
Start itrafungol 5mg/kg PO SID x7d until 5/13 (alternating weekly treatment plan-treat 5mg/kg PO SID on weeks 1, 3, 5 and re-test)-must treat for 2 negative DTMs
Start metronidazole 15mg/kg PO BID x5d until 5/11 (first dose given)
Start lime dips twice a week (first given today and scheduled next one for 5/9)
Rec NH placement for treatment
Recheck 5/13-adjust dosing
Prognosis: Good to fair
SURGERY: temporary waiver due to ringworm, URI, and underage
